== Formation == Macrophages are phagocytic cells that are produced during an injury or infection. They defend against infectious microorganisms, but also play a role in homeostasis and wound healing. Fusion also requires the presence of DC-STAMP, which is a transmembrane protein, and E-cadherin, CD206, MFR, and CD47, which are different types of receptors. Fusion of these macrophages involves many other proteins, receptors, and molecules as well, but the ones previously mentioned are the most crucial. Finally, macrophages also use filopedia to assist in fusion through sharing cytoplasm between cells.

=== Peripheral edema and hypoalbuminemia === Kwashiorkor is a form of protein deficiency, which can result in both osmotic imbalances and irregularities in the lymphatic system. Kwashiorkor is most notable for peripheral edema. The presence of edema in kwashiorkor is correlated with very low albumin concentration (hypoalbuminemia). Edema results from a loss of fluid balance between the hydrostatic and oncotic pressures across the capillary blood vessel walls due to the lack of protein which affects the body's ability to draw fluid from the tissues into the bloodstream. Low albumin concentration influences negatively the strength of oncotic pressure. Failure leads to fluid buildup in the abdomen, resulting in edema and belly distension. Furthermore, the release of antidiuretic hormone is stimulated by hypovolemia, also leading to the development of peripheral edema. Plasma renin is also stimulated, promoting sodium retention. It is important to distinguish the pathophysiology of marasmus and kwashiorkor when it comes to treating malnourished children who may have hypovolemic shock that is caused by an acute loss of salt and water. Children with severe albumin deficiency struggle physiologically to maintain their blood volume.

When a useful activity has been identified, chemists will make many similar compounds called analogues, to try to maximize the desired medicinal effect(s). This can take anywhere from a few years to a decade or more, and is very expensive. One must also determine how safe the medicine is to consume, its stability in the human body and the best form for delivery to the desired organ system, such as tablet or aerosol.

Pelage Pharmaceuticals presented on suvomipic at the American Academy of Dermatology (AAD) Annual Meeting 2026 in March 2026, including disclosing some new phase 2a trial data, but did not provide the open-label extension data at this event. Phase 3 trials of suvomipic for alopecia are being planned for 2026. It has been estimated that if suvomipic is successful in clinical trials and passes regulatory scrutiny, the drug could be approved for the treatment of hair loss sometime between 2027 and 2029 at the earliest. Suvomipic is currently Pelage Pharmaceuticals's only official drug candidate. However, the company is also investigating reactivation of dormant stem cells for treatment of other medical conditions. Pelage Pharmaceuticals had raised more than $30 million by August 2025, with investment being led by Google Ventures of Alphabet. In October 2025, it was announced that the company had raised $120 million. In 2019, Pelage Pharmaceuticals had reached an agreement that granted Allergan (now AbbVie) an option to acquire the startup company, but this deal was terminated in 2020. Suvomipic may also have applications in the treatment of other forms of hair loss besides androgenic alopecia. In October 2025, Time magazine named suvomipic one of the best inventions of 2025. === Pharmacokinetics === GBL is rapidly converted into GHB by paraoxonase (lactonase) enzymes, found in the blood. Animals which lack these enzymes exhibit no effect from GBL. GBL is more lipophilic (fat soluble) than GHB, and so is absorbed faster and has higher bioavailability. Because of these pharmacokinetic differences, GBL tends to be more potent and faster-acting than GHB, but has a shorter duration; whereas the related compound 1,4-butanediol (1,4-B) tends to be slightly less potent and slower to take effect but longer-acting than GHB.

A significant limitation to paper chromatography is that it will not show exactly how much malic is still remaining in the wine, with the size of the "splotch" on the paper having no correlation to a quantitative figure. The sensitivity of the paper is also limited to a detection threshold of 100–200 mg/L while most measurements of "MLF stability" target a malic level of less than 0.03 g/L (30 mg/L). The enzymatic method allows for a quantitative measurement of both malic and lactic acids, but requires the expense of reagent kits and a spectrophotometer that can measure absorbance values at 334, 340, or 365 nm.
